Coverage Is Only the Beginning: Expanding Home Accessibility for Anthem Medi-Cal Members Across California
When mobility changes, a home can stop working the way it once did.
A bathtub becomes difficult to enter safely. Stairs become harder to navigate. A doorway may no longer accommodate a wheelchair. These are physical details, but they can determine whether someone is able to remain safely and independently in the place they call home.
Through California’s Medi-Cal Community Supports initiative, Environmental Accessibility Adaptations (EAA), or home modifications, are designed to address these kinds of challenges. For eligible members, covered EAA services are generally payable up to a $7,500 lifetime maximum, subject to applicable eligibility, authorization, benefit requirements and exceptions.
Rosarium Health has contracted with Anthem to help eligible Medi-Cal members access and complete these home modifications across California.
“We want eligible Medi-Cal members who need home modifications to be able to use the benefits available to them,” said Alana Pfeffinger, Regional Program Manager, CalAIM. “Working with providers like Rosarium Health helps members and the care teams supporting them take the next step, from identifying a need to making the home safer and more accessible.”
Coverage Is the Starting Point
Having a benefit available is important. But coverage alone does not install a grab bar, widen a doorway, or build an exterior ramp.
Between identifying the need and completing the work, several things still have to happen. A member may need an in-home clinical assessment. Documentation has to be completed. The benefit may require authorization. A qualified local contractor has to be identified and scheduled. Once the work is finished, the project still needs to be verified.
That is the middle piece Rosarium handles.
We connect health plans, care teams, clinicians, local contractors, and members so that an identified need can become a completed home project.
Early Evidence on the Impact of Home Modifications
California’s experience with Environmental Accessibility Adaptations is showing promising results.
A 2025 analysis by the California Department of Health Care Services evaluated Medi-Cal members who received Environmental Accessibility Adaptations through Community Supports. In the six months following their home modifications, DHCS found that:
- Inpatient service costs dropped by 55.4%
- Emergency room costs dropped by 43%
- Long-term care costs decreased by 8.7% per member per month
Within Rosarium’s own population, 77% of eligible members who receive an in-home assessment go on to complete a home modification.
These findings suggest that appropriately supporting people in their homes can be associated with lower acute-care and long-term-care utilization while also helping people maintain their independence at home.
How Rosarium Supports the Journey
Rosarium manages the full lifecycle of home modification coordination so members and care teams have one clear process from an identified need through completed work.
- In-Home Clinical Assessment
We coordinate in-home clinical assessments to understand the member’s functional needs and identify appropriate modifications for safety and independence. - Documentation and Authorization
We coordinate the documentation and benefit authorization process in collaboration with health plan partners. - Qualified Local Contractors
We identify and deploy qualified local contractors to complete the approved work safely, efficiently, and to a high standard. - Project Completion and Verification
We manage the project through completion, including quality review, documentation, and contractor payment.
Healthcare can do a strong job identifying when someone needs support, but the process often becomes fragmented once the solution involves changing the home. We built Rosarium to manage that follow-through, from assessment and authorization through local delivery and project completion.
As Rosarium’s Founder and CEO, Cameron Carter, explains:
“Healthcare can do a great job identifying when someone needs support, but too often the process becomes fragmented once the solution involves the home. The real challenge is the follow-through: navigating assessments, approvals, local delivery, and construction without leaving the member or care team to manage it on their own. We built Rosarium to bridge that gap and create the end-to-end support needed to turn an identified need into a completed change in the home. Working with Anthem gives us the opportunity to bring that support to more Medi-Cal members, and we are focused on earning that opportunity one household at a time.”
Serving Communities Across California
Through this partnership, Rosarium is expanding its reach across Anthem’s California service area, including Los Angeles, Sacramento, San Francisco, Santa Clara, Fresno, Kern, and surrounding regions.
With that scale comes responsibility.
Whether the need is ramp access, a safer bathroom, a wider doorway, or another medically necessary change to the home, our focus is the same: deliver the work reliably, support the member and care team throughout the process, and earn the opportunity one household at a time.
