When a Member needs medically necessary home modifications to remain safely in their home, contact Rosarium. Assessments are performed by a specially trained Physical or Occupational Therapist. Contractors have training and understand the unique needs of this vulnerable population as they evaluate the home, prepare the bid and complete approved home modifications. The result is a satisfied Member and a positive outcome.

How it Works

The network

Rosarium offers a network of credentialed clinicians and contractors. These network providers are independent providers, not a Rosarium employee. A licensed occupational or physical therapist assesses the home and provides the recommendation for modifications based on the clinical and environmental evaluation. A licensed, insured, and bonded contractor provides the bid and completes the approved work.

What sits with the health plan

Rosarium handles the referral from initiation through completion of a job. When the health plan receives a prior authorization, we make it easy. All the required documents and information required to approve there, minimizing the time for your staff,

What sits with the member

We make it easy for the member and their family, taking away the unknowns of how to make their home safe, The member must be at home for the assessment, be available or designate someone to meet the contractor for the bid and then the approved modifications. Everyone can be assured that people who come into their home from the Rosarium’s network has been screened, with the proper credentials to provide services.

Rosarium receives a referral from the health plan, Care Manager family member, physician or may contact us directly. Once a referral is received, the Rosarium team works with the Member, referral source, health plan, network clinicians and contractors to move the project to completion in a timely manner. Once a job is approved by the Health Plan, the modifications can be completed within 30 days. Because Rosarium controls the end to end process, there is no fragmentation, the Member is satisfied with a quality job.
We have a team that diligently works to identify current and new referral sources affiliated with the Health Plan, such as participating Care Management companies, FQHC’s, hospital systems and community service providers. Rosarium builds strong relationships which results in more members receiving the home modification benefits they need and deserve.
Rosarium has a unique business model, unlike any other home modification provider. Our credentialed, independent network of specially trained physical and occupational therapists and contractors work together as a team, along with our internal clinicians, to ensure the quality outcomes the health plan expects of their network providers.
Rosarium has network providers in 32 states. When we sign a contract with a health plan, our network team evaluates and enhances our network based on the health plan’s service area. We are continually monitoring the network to meet network adequacy standards.
Rosarium Health supports multicultural and multilingual member populations by matching members with a culturally reflective provider network and embedding cultural humility throughout the home modification process. We recruit clinicians and home modification contractors who mirror the demographic diversity of the plan’s local membership.
