Referral Received
A care manager or provider sends us a referral (online or by form).

A licensed OT or PT from our network visits the member's home to assess safety needs and functional risks.
Care Plan + Approval
We partner with you to create a detailed home modification plan and handle documentation and approvals with the plan.
Installation
A local, licensed contractor completes the modification — from grab bars to full bathroom updates.
Completion + Follow-Up
We close the loop with photos, documentation, and status updates for your records.
